CGE is working in concert with OSU administration to begin to address some of the many safety issues present across campus.  OSU has asked us to reach out to our membership to generate a list of specific safety issues that need to be addressed.  Please use the form below to report any safety issues of which you are aware.  Including the specific building, lab, room number, and any other relevant information will help OSU in addressing these issues as quickly as possible.  All responses will be aggregated and anonymous.

Some examples of safety issues might include:
--Inadequate training (chemical safety, equipment use, first aid, etc.)
--Inadequate supplies (first aid kits, respirators, eye wash stations, etc.)
--Unsafe environment issues (unstable shelves and other fixtures, blocked doors and vents, inadequate chemical and equipment storage, etc.)
