Assumption of Risk:

I, the undersigned, acknowledge that participation in music classes involves inherent risks, including but not limited to physical injury, emotional stress, and personal property loss or damage, while engaging in activities such as playing musical instruments, attending class sessions, or participating in related activities. I voluntarily assume all risks associated with the participation of the named participant in the music class, whether such risks are known or unknown to me at this time.

Release of Liability:

In consideration of the opportunity for the named participant to participate in the music class, I hereby release, discharge, and hold harmless [Your Business Name], its owners, instructors, staff, and affiliates, from any and all claims, liabilities, or demands, including those arising from negligence, related to any injury, accident, damage, or loss that may occur during the course of the music class.
