PCSD 25 is committed to prioritizing schools where every learner feels safe,
respected, and valued. The universal consent form is designed to help the
district comply with state law with regard to parent consent. It is
important to understand that the law prohibits teachers from using
any name, other than the name, including nicknames, listed on the school’s
official record unless the school has received written permission from a
parent or guardian to do otherwise. The law also prevents schools from
providing medical treatment or any counseling services without written
consent. We understand that these issues are important to our families. As
always, our goal is to remain in compliance with the law while continuing to
provide high quality educational services in a supportive environment for all
learners and staff.
Responses to these questions will remain in effect unless the school receives modification requests in writing. This form will be provided to all parents/guardians prior to their child starting 6th grade, and 9th grade as new middle and high school consent questions apply, including permissions relating to Military Recruiters and Sex Education Topics.
