New Student Enrollment Form
We're so excited for you child to join the MTA family!

This form will help us start the process of enrolling your child in the 2026-2027 school year. 

After clicking "Submit" on this form, you will see a link to our online payment system, Square, that gives you the ability to pay your deposit online. Unfortunately, we are unable to guarantee your child's seat until the deposit is collected. If you wish to pay your deposit in person, please do so as soon as possible to secure your seat. 

From there, you will receive documents from DocuSign that will finalize the enrollment process.
