At this time we are asking for a commitment of the first semester from families who are choosing the online learning option for their student(s).  This form will serve as your enrollment into the online learning program.  Please complete by Friday, August 14, 2020 @ 4:00 p.m.  After your enrollment, you will be sent information on how to access the online learning and school supply list from the online teachers.  

Please complete the form for each child that will be attending New Vision Charter School through the online learning option we are providing as an option in the fall.

IF YOU ARE NOT CHOOSING 100% ONLINE INSTRUCTION FOR YOUR STUDENT IN FALL OF 2020 DO NOT COMPLETE THE FORM. THIS FORM IS TO BE COMPLETED ONLY BY FAMILIES THAT ARE CHOOSING ONLINE INSTRUCTION REGARDLESS OF THE SCHOOLS CURRENT PLAN TO PROVIDE FACE TO FACE INSTRUCTION WHEN SCHOOL OPENS IN AUGUST.
