SBA Table Request Form 
Thank you for your interest in using the SBA tables for a school, club, or organizational event. Please understand that this is a table request form, and is not a confirmation for the reservation. After fulfilling this request form, please email sbauofa@gmail.com and cc the current SBA Secretary (andrebengtson@arizona.edu) for the next steps of the request. 

Upon approval, SBA will respond to your email and confirm a time for pick-up and drop-off of the tables. Only then will the tables be confirmed for your event. Please ensure that the information provided below is accurate to ensure a rapid response so SBA can be assist your event.
