This form will be forwarded to the psychology department chair and your independent study advisor, who will notify the department Administrative Assistant (Christine Mennillo) when your ind. study has been approved. Ms. Mennillo will send your approval to the registrar's office. You will receive registration confirmation from the registrar when it's complete.
