Surplus Equipment Form
Use this form to submit any equipment that is no longer in use in your building.  Please fill out as much of the information as possible.  Submit the form once for each piece of equipment.

Is the equipment an old Mac or Standard Surplus (other computer, printer, etc...)?
Asset
If the equipment still has a visable asset tag, please provide the tag number.
Serial
Provide the serial number, if known.
Description *
Briefly describe the condition of the equipment or why it is being added to surplus.
School *
Select your school.
Location *
Is the equipment in the process of being moved to central office, or does it need to be picked up?
Pickup
If the equipment needs to be picked up, please specifiy a location in your building.  
Submit
Clear form
Never submit passwords through Google Forms.
