Feedback form for open-ended youth consultations
This form is used to give feedback and report outcomes from open-ended consultations. This is an important step for youth priorities to be recognized, as the outcomes of open-ended youth consultations will feed in to UN MGCY policy.

Please note that a participant list with specified age of the youth is preferably sent to info.whs@childrenyouth.org, along with pictures and any other material you wish to share. You are also welcomed to send a short summary from your consultation to the list whs@childrenyouth.org.  One feedback form per consultation is preferable.
