Informed Consent
A research project on physics education is being conducted by Stephanie Friend, a student in the Department of Liberal Arts and Engineering Studies at Cal Poly, San Luis Obispo, and supervised by faculty advisors Dr. Michael Haungs, Dr. Eric White, and Dr. Sandrine Fischer White.  The purpose of the study is to assess visualizations that analyze physics problem sets and equations.  You are being asked to take part in this study by answering a series of questions about these visualizations.  Your participation will take approximately 5-15 minutes.  Please be aware that you are not required to participate in this research and you may discontinue your participation at any time without penalty. You do not have to answer any questions that you choose not to answer.
The possible risks associated with participation in this study include very minor psychological risk.  If you should experience emotional distress, confusion, or low self esteem, please be aware that you may contact people for help. If you are a student, contact the Cal Poly Health Center at 1-805-756-1211. If you are a faculty member, you may contact the Employee Assistance program at 1-800-367-7474. Your confidentiality will be protected. If you volunteer for this research, you will have the option to select different levels of confidentiality and omit your name. Potential benefits associated with the study may include an improvement to physics education.
