Membership Sign-Up Form

NEW MEMBER SIGN-UP:
Please enter information for (each of) your business(es) below.

MEMBERSHIP RENEWAL:
If you're a current member and simply renewing this year - besides your name, you may type the word "SAME" in the fields on the form and we'll keep your current info the same.

INVOICING:
You will also be sent an email and/or printed invoice with your total amount due, and details on how to pay your dues. You may pay via PayPal or by mailing in a check. If you would like to pay via credit card/Pay Pal – please email Amanda Amanda@northwoodshardware.com Each invoice will identify the dues amount based on the 2015 dues based on the number of businesses being renewed, please adjust if needed.

IMPORTANT:
If you have more than one business, you will need to fill out a second form. Once you submit your first form, you will have the option to click on a link to submit another form.

IMAGES:
Please submit up to three photos/logo graphics per business membership. Please email high-resolution jpg or png files to our Marketing Director, Raquel Jackson: rockwellart.design@gmail.com These images will be used to represent your business on the website, in our social media, and may be used for other community marketing purposes!
