Iredell Museums may take photographs during programs or tours for usage in advertising, marketing, and social media. Please let us know if you do not want photographs taken in the comment box at the bottom of the form. 

I understand that participating in camp activities is potentially hazardous, and that I should not register my child unless he/she is medically able. I assume all risks (known and unknown) and assume full responsibility for my child’s participation. In case of an emergency I hereby authorize my child to be treated by Certified Emergency Personnel (i.e. EMT, First Responder, and/or Physician). Iredell Museums is not responsible for lost or damaged personal property. All scheduled events are subject to change. I understand that no fees will be refunded or transferred unless a child is unable to participate due to an accident or illness per physician orders.
