We regularly host coding, CAD, and art camps for younger students and actively participate in local cultural events and environmental improvement projects, demonstrating our commitment to the community. On the first page of the form, fill in all large blanks with "The Alliance of Youth Leaders in the United States (aylus.org)" Fill in the "State of ______" blank with "Washington" And fill out Date, Signature, and Print Name accordingly. For the second page of the form, fill blanks based on the labels:
(Agency)：Fill in "AYLUS Lake Washington Branch"
(Program): Fill in "all the programs that I choose to attend".
(Dates) : Fill in "[Current date] to [Aug.2028]"
