WMMMS - Off Campus Professional Learning Request Form
Prior to enrolling or being placed on a waiting list for any professional learning (district mandated, departmental, or renewal), you are required to complete this form within 15 days of the scheduled request leave. Failure to complete and submit this form (your request) may result in a denial of such leave.

The eSchools manager will register you for all school based professional learning sessions listed on our Professional Learning Plan to include Early Release Days and (required) Planning/Afterschool PL.

After submitting this form, an administrator will contact you regarding the approval status of your request.
