Jewish Family & Child Service (JFCS) wants to plan programs and services that fit your life. You know best what helps. We want to hear from you.

Please fill out the survey below. Your feedback will help us better serve older adults. This survey is for you if you are an older adult, if you help care for one, or both. It is also for you if you have never used JFCS before.

Before you start:

It takes about 7 minutes.

We do not ask for your name unless you choose to give it at the end.
Your answers will not change any services you get from JFCS.
