Thank you for your interest in adopting a cat! To advocate for responsible pet ownership, this form is intended to help you think through all of the necessary considerations and arrangements you would have to make before committing to adopting a cat. 

For information, the cats that are up for adoption are stray cats rescued by me and have gone through basic health check-ups and treatment for fleas and worms. I may request regular updates and photos in the first month and I reserve the right to take back the cat at any time I feel it is in the best interests of the cat. Do you agree to comply? *
I will need proof of vaccination and spaying/neutering (for cats not yet spayed/neutered) as per the required schedule. I will also be collecting a deposit at date of adoption which is fully refundable to you once you have shown me proof of spaying/neutering. Do you agree to comply? *
As part of the adoption process, I will request a video tour to be sent of your home to ensure your home is safe, clean and cat proofed. Do you agree to comply? *
Are you prepared to handle a settling-in period, which may include behaviours such as hiding, excessive meowing (especially at night), reduced appetite, litter box accidents, or reluctance to interact? *

By signing this form, I commit to giving this cat a loving, safe, and lifelong home. I understand that adoption is a responsibility that extends for the entire life of the cat, and I will provide all necessary care (such as regular checkups and dental health maintenance), nutrition, and medical attention this cat requires. *
By submitting this form, I consent to the information provided being shared with the owner or rescuer of the cat. I understand that my personal details will be kept confidential and will not be shared with any other parties. This consent is given in accordance with Malaysian laws governing the protection of personal data, including the Personal Data Protection Act 2010 (PDPA).
