Hello!
This form is to assist in gathering information for us to call you for setting up a preschool evaluation for students ages 3 to 5, though we can take your information over the phone at (435) 835-2271 or the preschool directly at (435) 835-2511. Our schools are primarily funded from Special Education but we do take some tuition students. Providing your student's information to this form will get your child on the evaluation list for the next available testing date or for consideration for one of the tuition spots. If you have children ages 0-2 who may require a disability evaluation and possible services, please contact Central Utah Public Health Department Early Intervention at (435) 896-5451or visit their website at
https://centralutahpublichealth.org/home-visitation/For the 2025-2026 school year, initial preschool testing will be August 18th with the first day of Preschool starting Tuesday, September 2, 2025. Subsequent testing dates throughout the school year will follow. Please check the website or preschool teacher for those dates. You may reach out to Manti Elementary Preschool Teacher Krisell Hill at (435) 835-2511 or email at
krisell.hill@ssanpete.org for further questions.
If you have needs requiring further assistance and direction in this process, please feel free to contact Special Education Director Aaron Peterson at (435) 835-2261 or email at
aaron.peterson@ssanpete.org We look forward to hearing from you!

Special Needs Opportunity Scholarship Grants
The Carson Smith Special Needs Scholarship provides tuition assistance for
qualified students with disabilities enrolled in eligible private schools. The
scholarship is for students who would qualify for special education and related
services in public schools, preschool through 12th grade (3‐21 years of age), whose
parents choose an eligible private school.
The Special Needs Opportunity Scholarship Program is an education choice model
for Utah residents that provides tuition assistance for eligible students with
disabilities not enrolled in public school. It is designed for students who would
qualify for special education and related services in public schools, kindergarten
and grades 1 through 12 (5–21 years of age), whose parents choose an eligible
private school or other eligible expenses.
