This is the google form needed to become a tutor through Foster Futures Together. In order to join this program, students must be within grades 8-12, have a passion to give back to the community, and commit to hosting weekly sessions with their students, although individual scheduling and subjects taught can be chosen at their discretion. Students who are selected for this program will receive an email notification within a few days.

 I understand and acknowledge that this release discharges Foster Futures Together and its affiliates from any liability or claim that I may have with respect to the services provided by Foster Futures Together. I understand that Foster Futures Together does not represent its assigned tutees and that this is a virtual, online-only tutoring program. Foster Futures Together does not recommend or require that tutees and tutors meet in person and therefore, this release extends to all bodily injury, personal injury, illness, death, reputational damage or property damage that may occur or arise if the tutors and tutees meet in person, outside of the virtual, online-only tutoring relationship.
 
 I understand that my course materials, lessons, advice, or guidance are my own and release Foster Futures Together from any culpability associated with negative results.
 
 I expressly agree that this release is intended to be as broad and inclusive as possible under the laws of the State of Arizona and that this release shall be governed by and interpreted in accordance with the laws of the State of Arizona. I agree that in the event that any clause or provision of this release is deemed invalid, the invalid portion shall be disregarded to the minimum extent possible and the enforceability of the remaining provisions of this release shall not be affected and shall remain in full force and effect. *
