INTRODUCTION: This is a suicide prevention training. This document defines the terms and conditions for giving your permission to allow your child to participate in this training,
DESCRIPTION OF THE TRAINING: Your child will be asked to participate in suicide prevention training. The purpose of suicide prevention training is to provide information about suicide prevention, how to identify when a classmate, friend, or relative may be at risk for self-harm, and how to talk to adults to help that person.
RISKS AND BENEFITS: Research has shown that talking about suicide does not cause people to hurt themselves. However, in the event that your child becomes upset, the trainer will recommend that your child talk with qualified school staff. There is a risk that participants may experience some emotional discomfort from the discussion of suicide. There is potential risk of losing privacy/confidentiality. A benefit from your child’s participation in this training is learning that suicide can be prevented, learning the risks and warning signs that could help your child identify and help someone s/he believes is having thoughts of suicide, as well as learning about the school and community resources that are available to young people.
CONFIDENTIALITY: Trainers will encourage students to respect each other’s privacy. However, if your child participates in this training, you and your child should be aware that is s/he shares something personal during the discussion, there is a possibility that it may not be kept confidential by other students. If your child tells the trainer that s/he plans to hurt him/herself or others, the trainer will need to refer him/her to help immediately. Bullying is not tolerated and if your child reports bullying to the school staff the school will follow their plan for addressing the bullying.
CONTACTS FOR QUESTIONS ABOUT THE TRAINING: You may contact George Meiwes, SJN Principal with questions about the training at principalsjn@sjnok.org. I have been provided with the link to access materials used in the classroom during the training.

I understand my child’s participation is voluntary and that I may withdraw my child at any time without penalty.
