CMS Baseball 2026-2027 Tryout Form
CMS Baseball will host TRYOUTS for Spring 2027 on Tuesday, June 12, 2026, from 6:00 - 8:00pm. In case of weather, the makeup will be Thursday, June 18, 2026, at the same time and place. 

Additional tryout opportunities will be held in September and again the week of January 31st, 2027. 

However, NOW is the time if you are available! 

Please fill out this form BEFORE arriving at the Clinton High School Baseball Field at Jaycee Park/Fairgrounds that evening. 

Please plan to arrive at the field, dressed in baseball pants, numbered shirt or jersey (of any kind), and hat (of any kind) 15 minutes early! 

BRING ALL BASEBALL EQUIPMENT, including cleats, turf or tennis shoes, bats, gloves, etc. Catchers bring ALL personal gear.  

This team is SELECTIVE. Not all players trying out will make the final cut. The information you provide here will help us know who is whom during the tryout. 

Players who make the cut for this tryout will be invited to Fall Practice as well as Winter Workouts (strength, conditioning, and throwing programs). Dates and Times TBD. 

We encourage all players to continue personal trainings/lessons and to continue playing on recreational or travel teams through the fall. Players are also allowed to play travel ball throughout the spring season, though practices and games may conflict with our program, which should take priority in-season.

*Both parent and student MUST have a Final Forms account. Both parent and student MUST login and complete ALL online forms. Use a NON-ACS email address for the student (gmail, yahoo, icloud, etc.) when prompted. 

The final step to being compliant is to upload a scanned copy or photograph of the signed and dated MEDICAL ELIGIBILITY or CLEARNANCE FORM in the PPE Physical Packet found on Final Forms, which is to be printed and taken to the healthcare provider as the last step in Final Forms for the 2026-2027 school year.
