I acknowledge that COVID19 is a contagious virus and that public health authorities are highly recommended social distancing, sanitisation and other preventative measures to mitigate the risk of spreading/contracting COVID19. Upon the recommendations of these authorities, Get Wet Co Pty Ltd is also ensuring and enforcing these preventative measures, including but not limited to social distancing. *
I further understand and acknowledge that Get Wet Co Pty Ltd cannot guarantee that I will not become in contact/contract COVID19. I understand that contracting/coming in contact with the virus may be because of negligence, actions or omissions of myself or others, including but not limited to Get Wet Co Pty Ltd staff and customers. *

I agree to the following - I am not experiencing any symptoms of a respiratory illness or COVID19 including but not limited to shortness of breath, dry cough, fever, loss of taste or muscle and head aches. I have not travelled internationally within the last 14 days. I have not. I have not been knowingly exposed to or have been confirmed to have coronavirus within the last 14 days. I am not meant to currently be in any form of quarantine or isolation *
