A Google/GMail account is not required to complete this form but logging in allows you to save or edit your submission(s). Nominators may submit more than one individual for consideration and will receive a copy of their submitted form.

Incomplete submissions will be rejected owing to time and the large number of expected nominations.

All personal information provided to the Member will be treated as private and confidential. This Information will not be shared with any other individual or organization save for the Chancellery of Honours at Rideau Hall in Ottawa.

The 20 successful Nominees and their Nominators will be notified in the late October timeframe. Medals will be presented to the recipients at an event in Oshawa later this fall.
