Future Vote Initiative: Parent/Guardian Authorization Form
The Future Vote Initiative (FVI) is a year-round program for 6th-12th grade students to learn about elections and civic participation.

A 1-hour training class is mandatory in order for your student to work at a polling place on Election Day.

Both you and the student are required to attend the entire class.

As parent/guardian, you are responsible for transportation of your student on Election Day to and from the assigned polling place. You are responsible for transportation of your student when they participate in other FVI activities throughout the year.

If eligible, you are expected to register to vote or update your voter registration as well as encourage other eligible family members to register or update their voter information.

As parent/guardian, you must authorize the student to participate. Please provide the following information so your student can join the program. All the information you submit is used solely by the election office.

In consideration for being allowed to participate in Future Vote Initiative (FVI), I release from liability and waive my right to sue [insert election office/government office], their employees, officers, volunteers and agents from any and all claims, including claims of negligence, resulting in any physical injury, illness (including death) or economic loss my student may suffer or which may result from their participation in FVI, travel to and from FVI activities, or any events incidental to FVI.
