This form is designed to simplify the process of raising cases with Capita/NHS England regarding GPs' pensions information, and of subsequently escalating those cases to the Information Commissioner's Office (ICO) and the Pensions Ombudsman.
It asks you to give permission for us to:
(1) Submit a Subject Access Request (SAR) on your behalf to NHS England requesting your pensions data.
We will store these up and send them in a batch if we don't get a satisfactory response from NHSE on the requests we've made in our open letter to them.
This will mean that you get a copy of the information that Capita hold relating to your pension - neither GP Survival nor NASGP will see that data, and completing this form does not give us any right to see your pension information.
(2) Raise internal complaints on your behalf within NHSE if you are not satisfied with the response you receive.
This will enable us to see the scale of errors in people's pensions data, and to raise these collectively with NHS England and in the press.
(3) Raise cases with the ICO and the Pensions Ombudsman if needed.
