About this form:
This feedback form is divided into multiple sections. First to collect university affiliation, then to collect responses related to process, the phase one review of administrative programs, phase one review of academic programs, and lastly an opportunity to submit short, general feedback and comments.

The form was created by University Relations in conjunction with the Office of Academic Affairs and Research and the University President to help guide the Strategic Pathways process and provide opportunity for input from the university community.

Collected responses will be shared with university leadership and relevant working team members without aggregation, so responses should be brief and non-confidential. Participation through this form is optional.
