for the term from 1st of October 2023 to 30th of September 2024.

To affiliate activities with IFMSA Programs, the following documents need to be submitted

• A filled-out candidature form, signed and stamped by the President or an authorised representative of the National Member Organization of the Activity Coordinator. 
• A filled Activity Enrollment Form online. 

By signing this candidature form, all National Member Organizations involved in the activity enter into liability for any damages caused by their activity after being affiliated to an IFMSA Program. This applies to the entire timeline of the Activity.

The use of information provided in the form will be in line with your response in the form and will not be used for any other purpose. 1 The President of the National Member Organization (NMO) of the Activity Coordinator can authorize a member of the NMO to approve Activities in their stead, by naming an authorized representative in the NMO Report.
