LAVI Program: Service Details & Other Information

Description:

Lasting Autonomy in Valued Individuals (LAVI) Program, LLC, is a self-enhancement service dedicated to individuals, and their loved ones, in need of additional guidance and support with navigating their day-to-day lives and spiritual journey. LAVI Program assists in the promotion of personal growth and enhancement; access to community resources and connections; improving social interaction and personal relationships; strengthening interpersonal skills and awareness; and navigating present areas of concern in one's life.

**Tarot Readings DO NOT require a deposit, but MUST be paid in FULL.**

Appointments/Cancellations/No Shows:

  1. Scheduling for an appointment will be solely completed through a third-party booking site. Scheduling details will be provided by LAVI Program to access appointment availability, pricing information, payment processing, and cancellations.
  2. Appointment cancellations require a 24-hour notice from the date and time of the scheduled appointment booked.
  3. A $40.00 deposit will be charged in the case of a No Show or Cancellation not meeting the 24-hour cancellation requirement.
  4. Appointments are done virtually. A secure direct link will be sent to the email address provided for confirmation of booked appointments, acknowledging receipt of the link sent.
  5. Scheduled clientele will be allotted a 10-minute grace period after the scheduled time of an appointment to account for unexpected minor technical difficulties (e.g., audio, video, login, internet connection, etc.). Exceeding the 10-minute grace period will result in retaining the $40.00 deposit.

Payment:

  1. Payments will be completed through the same third-party site used for booking appointments.
  2. Payments will also be accepted via CashApp to $LAVIProgram. In the “for” section, please identify the service scheduled for.
  3.  Payment must be received to confirm the appointment slot being sought.

Booking Deposits:        

  1. Scheduled appointments for Life Coaching Services  will require a $40 deposit to process the appointment. The remaining amount will be charged upon the completion of the scheduled appointment.
  2. No-Show (-) Fee = $40.00 deposit amount paid

Privacy Policy:

  1. Virginia law requires that life coaches obtain written authorization before releasing any identifying information about their clients. This includes obtaining authorization from the client or their authorized representative. While LAVI Program is not bound by HIPAA due to not being a healthcare practice, we will uphold the confidentiality of each client to respect individual privacy.

Legals:

  1. LAVI Program is NOT a healthcare practice.
  2. LAVI Program is an "out-of-pocket” expense that is NOT covered by medical insurance.
  3. LAVI Program is NOT a legal support agency. We do NOT provide legal consultation, guidance, or advice.
  4. LAVI Program does NOT provide any form of "treatment" or "diagnosing" service.
  5. The owner and operator of LAVI Program is a licensed Qualified Mental Health Professional (QMHP) with the Virginia Board of Counseling since 2019.
