Becoming Be A Mentor Certified

Step 1: Please read these FAQs about why and what you need to do.

Step 2: Create an account in BeAMentor.org

Step 3: Complete the requirements as shown on the screenshot below. To be an overnight chaperone, you need to be cleared through Level 3, which means every item on the list must be completed.

The Criminal Check, FBI Clearance, and Fingerprint requirement is met by completing a LiveScan fingerprint at a local UPS. You will download the LiveScan form (or click the little arrow on the right of “Fingerprint” to access the download form as well). The fee is covered by BeAMentor. Also under “Fingerprints” is a list of local UPS stores where you can have the LiveScan completed. Be sure to ask UPS for a copy of the LiveScan once completed. It should only take a day or two for the LiveScan to clear once you have emailed/faxed the form. All of the other items can be completed fairly quickly at home.

TB test: If you are at low risk for TB, you may opt out of the test by having a health care provider complete this form for you to upload. The upload link for all documents is on the right hand side of the page after you log in.
