2012 Registration Terms and Conditions

• 2012 season registration opens on April 1, 2012

• Communication regarding in-person registration days is done via the email list, backpack flyers when possible, road-side signs, and on our website – www.berwicksoccer.com.

• Registration is open from April 1, 2012 through June 30, 2012; registration fees are $35 for the first child and $25 for each additional child.

• If the registration fees cause a financial hardship, scholarships are available through the President at Berwicksoccer@gmail.com, via our Facebook page, or 207-956-0753.

• In-person registration times are as follows: Noble Middle School on May 7th at 6-8p, May 9th at 4-6p, and Saturday May 12th at 9:30-11:30 at the Berwick Recreation Field.

• Berwick Youth Soccer is not affiliated with the Berwick Recreation Department and completed registration forms cannot be dropped off with the Town of Berwick offices.

• Registrations need to be received in person or postmarked no later than June 30, 2012. Berwick Youth Soccer is not responsible for registrations and/or payments that are lost or delayed in the mail.

• Registrations received in person or postmarked July 1, 2012 or after are not guaranteed a spot on a team and will be placed on the waiting list. Players from the waiting list will be placed on a team in the event that a spot on a team has been vacated by another player. Players on the waiting list will be placed in the order that they are received by division.

• If your child withdraws before the second practice AND you notify the President of the withdrawal at Berwicksoccer@gmail.com or 207-956-0753, your registration fees will be refunded minus $10 for the team shirt. If any of these conditions are not met, you forfeit any refund(s) you may have been eligible for.

• Coaches and other volunteers do not have decision-making authority and NO REQUESTS should be made to them at any time. The board is not responsible for any guarantees made by any coaches, nor is the board responsible to honor any request made to coaches. You must contact a board member with any requests or issues that need to be addressed. You may contact the Berwick Youth Soccer board via email, phone, or Facebook. • We will do our best to accommodate practice night requests or any other requests; however, it is not within our power to grant all requests. Please notify the registrar of any requests by noting them on your registration form as soon as possible.

• If you make a special request (that is approved by the board) that should require your child to change teams after August 1, 2012 there will be an additional charge of $10 for a new team shirt that needs to be given to a board member immediately by cash or check.
