A child shall be excused from immunization for religious reasons upon signing of a notarized form by the parent/guardian stating that the child has not been immunized because of religious beliefs.

Failure to comply with this policy shall result in the child being excluded from school.

Immunizations may be obtained at no cost at the Manchester Health Department during its immunization clinics.
