Dear Superintendent:

This is a request for access to public records under section 149.43 of the Ohio Revised Code, the Public Records Act.

Under provisions of that statute, I am requesting copies of the following records in the possession of your district, whether maintained at the district or school level:

1. Records indicating the locations of "seclusion," "time-out," “calming” or “processing” rooms in your schools;

2. Board policies related to the use of the above-mentioned rooms;

3. Internal policies related to the use of the above-mentioned rooms;

4. Logs documenting the use of restraint and seclusion since the start of the 2007-2008 school year. (You may redact students' names from these logs.);

If the information is kept electronically, I would prefer to receive it in that format.

Should any portion of this request be denied, in part or in whole, I am requesting a written justification for its denial, along with a citation of the appropriate statutory exemption being cited for each record, or portion thereof, that is withheld.

I request that any copying or postage fees be waived. However, if you choose not to waive them, please contact me in advance with an estimate of those costs.

As you are probably aware, the law requires you to promptly prepare the records we are requesting and to make them available for inspection at all reasonable times during regular business hours.

I respectfully request your response within ten (10) business days.

Thank you for your prompt attention. If you have any questions about this request, I can be reached at YOUR PHONE NUMBER (optional) or EMAIL.

Sincerely,

YOUR NAME
