ASSUMPTION OF THE RISK, WAIVER OF DAMAGES, INDEMNIFICATION AND RELEASE OF LIABILITY BY ATTENDING EVENTS PRODUCED BY CTN 2022

The Creative Talent Network (“CTN”) encourages and requests all guests who attend events produced by the Creative Talent Network or Foundation  (“CTN”) to follow the most current guidelines issued by the Center for Disease Control and Prevention (“CDC”) for best practices to avoid influenza and respiratory illness such as COVID-19.

I understand that the President of the United States, the State of California, and the County of Los Angeles have declared emergencies at various times related to COVID-19. I further understand that COVID-19 is an extremely contagious respiratory illness and is believed to be spread by person-to-person contact as well as through the air and surface contact.

I understand that if I am not comfortable or cannot agree to execute this Waiver/Release then I may not participate in events produced by CTN.

I hereby acknowledge and assume the risk of becoming infected by COVID-19 by being at the CTN STUDIO or CTN EXPO. I understand that possible exposure to COVID-19 may result in the following: a positive COVID-19 diagnosis, extended quarantine/ self-isolation, additional tests, hospitalization (including possible medical therapy, Intensive Care treatment, need for intubation/ventilator support, short-term or long-term intubation, and other potential complications), emotional distress, loss of employment and the risk of death. In addition, I may need additional care that may require me to go to an emergency room or hospital. Further still, I acknowledge and assume all economic risks including, but not limited to paying for medical bills, the risk that I may lose income or be incapable of fulfilling my job responsibilities should I become infected.

I also hereby acknowledge and assume the risk that if I am infected with COVID-19, there is a chance that I could transfer and/ or infect others with the virus. I hereby agree to defend and indemnify CTN and its past, present and future agents and successions from all legal responsibilities or liabilities related to any claims that I, or anyone I infect, may allege against CTN related to allegedly contracting COVID-19 at the CTN STUDIO or CTN EXPO. I also understand and accept that there is inherent risk from a failure of other participants not following proper COVID-19 protocols, such as maintaining social distancing and hygiene measures, and that COVID-19 precautionary measures being employed by CTN or contraction of COVID-19 and suffering related injuries. I further agree that I will not attend events produced by CTN  if I experience symptoms that of fever, fatigue, difficulty in breathing, or dry cough or exhibiting any other symptoms relating to COVID-19 or similar virus at any time during the 14 days immediately prior to the commencement date of events at the CTN STUDIO or CTN EXPO. I will not attend events produced by CTN in person if I, or any member(s) of my household, is diagnosed to be infected with COVID-19 or a similar virus at any time during the 14 days prior to the commencement date of  an event produced by CTN.

I hereby expressly waive, and release CTN and its past, present and future agents and successors from any and all rights, claims, lawsuits or damages of any nature whatsoever arising from COVID-19 infection or transmission related to my participation in events produced by CTN. I understand and agree that this waiver and release is intended to be interpreted broadly in the favor of CTN.

BY ATTENDING THIS EVENT I UNDERSTAND THE ASSUMPTION OF THE RISK, WAIVER OF DAMAGES, INDEMNIFICATION AND RELEASE OF LIABILITY. BY ATTENDING EVENTS PRODUCED BY CTN, I AM RELEASING CERTAIN LEGAL RIGHTS I MIGHT OTHERWISE HAVE.
