Distance Learning Pass/No Pass Request Form 2020-2021

This form is to request a change of previously reported letter grades to Pass/No Pass for students who were in 11th or 12th grades in 2020-2021 as indicated by Assembly Bill 104. This should be completed by the student or parent/guardian.

It is important to note that some postsecondary educational institutions, including those in other states, may not accept a Pass or No Pass grade instead of a letter grade for admission purposes.

This form must be submitted by August 31, 2021.
