Applicant Instructions
- Log in to your OATH account.
- Note: Waiver applications are only available in the Medical Director's OATH account
- Click Applications on the left-menu:

- Click the Start, use the Filter by Services drop-down menu to select the agency for which you are applying for a waiver, click the View All Services Applications link:

- Click the Apply Now button next to the Scope of Practice Waiver - Agency application:

- Complete the application form and submit it for review.
Notes:
- The application will only be available for your agency after they have submitted an Organizational Profile Information application. If you do not see the application under the agency you selected, that agency needs to assign a gatekeeper who must submit an Organizational Profile Information application. Once that is done, a new Scope of Practice Waiver - Agency application can be submitted.
- If you previously started a Scope of Practice Waiver - Agency application, click the Continue button on the left navigation menu under Applications to continue working on it and submit it.
- Refer to the Approved Waiver Guideline PDF for the waiver type for which you are applying. The application will ask specific questions pertaining to that waiver type. The agency and Medical Director must meet all requirements as specified in the guideline.
- A waiver delegate may fill out the application once the second page has been saved. The waiver delegate may not fill out the Attestation and Submission page at the end. The Medical Director must check the attestations, sign and submit the waiver application.
