PROCEDURE FOR REFERRING A STUDENT

FOR EXCEPTIONAL CHILDREN’S

PROGRAM SERVICES CIA-P203

ANSON COUNTY SCHOOLS

  1. SCOPE:  
  1. This outlines the procedure ACS follows to refer a student for evaluation to determine eligibility for special education services. Any individual has the right to request an evaluation if it is suspected

the student’s difficulties may be caused by a disability.

  1. RESPONSIBILITY:  
  1. Parent
  2. General Education Teacher
  3. Exceptional Children (EC) Teacher
  4. Principal/Designee
  1. APPROVAL AUTHORITY:
  1. Exceptional Children (EC) Director or designee
  1. ACRONYMS AND/OR DEFINITIONS:
  1. Parent- Biological parent, step parent, grandparent, foster parent, legal guardian, or guardian
  2. Individualized Education Plan (IEP) Team – the parent/guardian of the child; one regular education teacher of the child; one special education teacher; the Local Educational Agency (LEA) representative; an individual who can interpret the evaluation results;  at the discretion of the parent(s) or the LEA, other individuals who have knowledge or expertise regarding the child;  the student, as appropriate and/or required
  3. IEP - Individualized Education Plan
  4. EC - Exceptional Children

  1. PROCEDURE:
  1. Referral is made in writing by parent or school staff for an evaluation by the IEP Team.

  1. EC Teacher sends an Invitation  to team members.
  2. IEP Team meets to complete the Initial Referral.

  1.  The IEP Team determines no disability is suspected.

  1. No evaluations will be conducted; Prior Written Notice is completed; EC process ceases; parent is provided Procedural Safeguards Handbook

  1. The IEP Team determines a disability is suspected and evaluations will be conducted.

  1. Consent for Evaluation is presented to parent
  1. Parent provides consent for evaluations

-Prior Written Notice is completed

-All screenings and evaluations are completed; any requested information is collected

-EC Teachers sends Invitation to IEP Team

-At IEP meeting, all information, screenings, and evaluations are reviewed.

-Eligibility Determination is completed

-Decision for IEP Team

5.3.1.1.1-Child is found eligible for EC Services; IEP is created; Consent for Services is presented to parent(s); Decision for parent

-Parent signs “yes” to Consent for Services; Prior Written Notice is completed; EC services are implemented as indicated on the IEP

-Parent signs “no” to Consent for Services; Prior Written is completed; EC process ceases; parent is provided Procedural Safeguards Handbook

5.3.1.1.2-Child is found not eligible for EC Services; Prior Written Notice is completed; EC process ceases; parent is provided Procedural Safeguards Handbook

    5.3.1.2Parent does not provide consent for evaluations; Prior Written Notice is completed; EC

    process ceases; ; parent is provided Procedural Safeguards Handbook

5.4        The IEP Team determines a disability is suspected and eligibility will be conducted using existing information; all existing information is reviewed; eligibility determination is completed; decision for IEP Team

        5.4.1 -Child is found eligible for EC Services; IEP is created; Consent for Services is presented to parent(s); Decision for parent

-Parent signs “yes” to Consent for Services; Prior Written Notice is completed; EC services are implemented as indicated on the IEP; parent is provided Procedural Safeguards Handbook

-Parent signs “no” to Consent for Services; Prior Written is completed; EC process ceases; parent is provided Procedural Safeguards Handbook

5.4.2-Child is found not eligible for EC Services; Prior Written Notice is completed; EC process ceases; parent is provided Procedural Safeguards Handbook
