*Varsity levels of these sports are available to MS students via placement tryouts. Competitive MS teams are not offered in these sports
Families, we are asking you to please purchase these uniform items prior to the start of your sport season via links sent to you from your coach.
Our certified athletic trainer, Nikolaus Holmberg, along with our strong partnerships with EmergeOrtho/Blue Ridge Bone and Joint and PT Solutions allows us to meet the needs of our large and active athletic community. CDS student-athletes will have access to our athletic training rooms in the Nash Athletic Center and Sgro Athletic Complex.
Prospective student athletes and families can prepare for the upcoming year early by getting the 24-25 athletic requirements completed over the summer and submitting them electronically (Magnus Health Portal) for approval before August 1, 2024. CDS is no longer accepting original paper documentation. NCISAA protocol states that all student-athletes are required to have a current Sports Physical on file prior to participating in any school sanctioned athletic practices, training or games.
Submit the NCISAA Physical Form via the Magnus Health Portal (middle school and upper school students only)
Action Items:
Any child in grades 5-12 that wishes to participate in the CDS athletic program must have an updated yearly pre-participation physical in their Magnus Health Portal. Students will not be allowed to participate in any CDS practice/match/game without this document. The form must be filled out by a physician, either the student’s pediatrician or at any local urgent care or minute clinic that completes Sports Physicals. This form does not substitute your child’s annual physical or wellness check. Completed physical forms need to be uploaded to Magnus.
As of 2021 Carolina Day School partnered with PT Solutions to provide a certified athletic trainer at all games and practices. Any student - athlete who will be participating in after school sports will need a signed consent form on file to be treated by our Sports Medicine team.
The form provides information for the student and parents on the rare occurrence of sudden cardiac death in youth athletes. All student-athletes and their parents must review the Concussion Awareness Act and sign and upload it to the Magnus Health Portal.
To increase and ensure the safety of our student-athletes, we have implemented a concussion management protocol following the Gfeller-Waller Concussion Awareness Act. All student-athletes and their parents must review the Concussion Awareness Act and sign and upload it to the Magnus Health Portal.
The NCISAA consent to participate and Medical Release Form, is required prior to participating in athletics for Carolina Day. The Consent to Participate Release Form must be signed prior to participating in athletic activities.
