#1 - State of Colorado Proof of Residency

Every year the Colorado Department of Education requires online schools to collect a Proof of Residency statement for our enrolled full-time students. Parents/Guardians are required to download, print and sign the CDE Residential Affidavit form linked above and return it to Jeffco Virtual - digital signature cannot be accepted. A list of other acceptable documents for Proof of Residency is below. If you do not have access to a device to photograph or scan your documents, you will be allowed to fax your form(s) to 303-982-9546 or send them back to JVA via US mail or hand-deliver it to Jeffco Virtual Academy 1505 S. Pierson Street, Lakewood, CO 80232.
Acceptable Documentation for Colorado Proof of Residency:
Property tax payment receipts – 2023
Residential Lease Agreement (current year 2023)
Rent payment receipts (most recent)
SNAP/TANF direct certification letter (current school year certification)
Utility service payment receipts showing service dates beginning 6/1/2023, service and mailing address in Colorado, addressed to the parent or guardian
Colorado Department of Education Residency Affidavit (Proof of Residency affidavit will be sent to our full-time student’s parents or guardians via email.) PLEASE NOTE: Digital signatures are not accepted on the CDE Form - Please add a signature and date and return.

Immunization Updates
Colorado law (Board of Health rule 6 CCR 1009-2) requires all students attending Colorado schools and licensed child cares to be vaccinated against certain diseases, unless an exemption is filed.
It is mandatory that families of Colorado students (remote or in-person) either:

Incoming Kindergartners are due for vaccines, and 6th graders require a Tdap vaccine - please see your student's care provider for updating their immunizations if you have an incoming Kindergartner or 6th grader. (or complete the exemption module if you would like an exemption for these vaccines).
Jeffco Student Fees
Fee Payment link: https://www.jeffcopublicschools.org/family_portal. JVAs only fee is the annual Technology Fee. All full-time 6th through 12th grade students have a fee of $50 because they are provided a Jeffco Chromebook and access to required online programs as part of the 1:1 Technology for Education program.

This profile is meant to be completed by the student, not the parent or guardian. JVA is designated as an Alternative Education Campus (AEC) by the Colorado Department of Education (CDE). This designation requires that a percentage of our students fall into at least one of the at-risk categories and helps us meet the needs of JVA students. Questions in this survey help determine the programming needed to best serve our students.
Admission into JVA is in no way tied to students having any at-risk factors. This information helps us serve students and families and all information remains confidential and will only be used to support student success.
