Organization Description: Black River Health Services, Inc (BRHS) is a Federally Qualified Health Center look-a like with three clinic locations and two pharmacies in Pender County. Manos Unidas is a health outreach program located within BRHS serving migrant and seasonal farmworker population through the provision of health services, care coordination, enabling services and health education. The goal of Manos Unidas is to improve the health of migrant and seasonal farmworkers and their families.
Position Description: Behavioral Health Specialist (BHS) is responsible for assessing farmworker needs including mental health services during outreach and mobile clinic visits. Needs assessments will be documented and reported to the Co-Program Director for referrals to cost friendly services. Behavioral Health Specialists will complete referrals by securing appointments with partnering agencies and provide appointment details with farmworkers prior to and the day of the appointment. BHS will continue to follow up with the farmworker to assess further needs.
Responsibilities:
- Complete Needs Assessments with H2A farmworkers in Southeastern NC
- Complete virtual outreach visits with seasonal and migrant families during off season (August- February)
- Provide health education on priority and relevant health topics including but not limited to Diabetes, Hypertension, Nutrition, Smoking, Cancer, COVID-19, Heat Stress
- Provide sufficient information for patients to utilize resources available to them including mental health, primary care, specialty care, etc.
- Assist patients as needed with referrals, including scheduling appointments, confirming appointment details and tech support for workers.
- Follow-up with patients who have screened positive and negative on RHS-15 depression screenings, have requested appointment with LCSW, or mental health provider, have had appointment with LCSW or mental health provider
- Enter encounters and health assessments into FHASES (Manos Unidas Database and Black River Health EMR for documentation, data analytics and tracking
- Work in correlation with UNCW SON Mental Health Services Feasibility Study intern, faculty and Manos Unidas interns on project development
- Report findings to Mental Health Services Coordinator and LCSW or mental health providers
Qualifications: Candidates must have excellent written and verbal communication skills in English and Spanish. In addition, candidates should possess the following:
- Bilingual Spanish/English
- Minimum High School Diploma with relevant experience. An Associate or Bachelor degree is a plus.
- Cultural competence with Hispanic/Latino populations including indigenous populations from Mexico and Central America.
- Interested in new initiatives.
- Ability to work independently and as part of a team.
- Ability to multitask and take on various projects simultaneously.
- Eagerness to contribute to the program's growth and success.
- Willingness to work late nights and occasionally on weekends.
- Willingness to work in difficult working environments including overcrowded, substandard housing, and adverse weather conditions.
- Current valid driver’s license
- Reliable Transportation
- Preference is given to those from farmworker families
