New Student Online Enrollment Process

 Student Documents

Please upload the following documents to your online enrollment application:

• Immunization record
• Birth certificate or passport
• Middle or high students: please upload student transcripts (6th-12th grade transcripts)

If your student is enrolling in TK/Kindergarten or 1st grade, please have the following forms completed and submitted to your resident school office. Click here to access these forms.

• TK/Kindergarten:

• Oral Health Assessment - is due by May of the first year in a public school for incoming TK, kindergarten or first grade students,  it is not due at the time of enrollment. For 2024-25 enrollees, this form is not due until May 2025
• The following are not required, but are highly recommended:

1. Health Exam - is recommended for TK and kindergarten students, but is not required
2. Developmental Readiness Survey - to be completed by your child’s preschool teacher.

1. If your child is not currently attending preschool, parents/guardians are welcome to complete this survey.
2. This document is submitted directly to your resident school office staff, after enrollment.
