Private Placement Service Plan Annual Notification

The Service Plan clones the information from the last Service Plan.

If the Information doesn’t clone, complete required information in the forms as appropriate for the student refer to steps below.

Note:

• The student's IEP is written as if the student was attending the school site. In the IEP enter Services and Supports that are offered at the school site.
• The Service plan date cannot be on the same day as the IEP plan date. The Service plan date must be dated after the IEP plan date.

• PPPSS services are only entered in the Service Plan.

Non Religious = For Profit

Eligibility should fill in from the student Profile.

• If eligibility data does load, enter data from the most recent IEP.

Complete with data from the most recent IEP.

Complete Services

Enter the exact data as listed below to complete the ‘Service’ details screen for service.

• Service Group, select PPPSS SP Services
• Service, choose one primary PPPSS service.
• Dates, If dates do not auto calculate, enter plan date for start date and + 364 for end date.
• Service Agency, select District of Service
• Service Delivery Method:, select PPPSS Consult
• Service Provider Title, select Resource Teacher-Spec Educ
• Service Provider, Optional leave blank
• School of Service, select Parentally Placed Private Schools
• Frequency, enter Yearly
• Duration,  enter 2 hours 0 minutes
• Location (Environment) select Any Other Location or Setting

Least Restrictive Environment (LRE) This is an automatic calculation.

Federal Setting

Select: Parentally Placed in a Private School.

Complete Annual Notification information

then create an Interim Placement to start the Interim process, to reinstate student back in District for SPED
