Health Requirements for School

Health Requirements
- Immunization records are due no later than Registration Day. Please review your child’s immunization history with your healthcare provider to ensure immunization requirements are up to date.
- All students need a complete physical exam carried out by a healthcare provider within 12 months of the start of the new school year. The physical exam should be documented by your healthcare provider on this school health form or this school health form for sports.
- Please submit a record of the most recent physical exam no later than June 2, 2025.
- D.A. requires all students to be fully vaccinated against COVID-19. Please include proof of vaccination with all other required health documentation.
Medication
- Students are not permitted to carry medications in their bag during the school day, except for certain emergency medications for asthma, diabetes or life-threatening allergies.
- If your child takes or may need to take prescribed or over the counter medicine at school a medication administration form is needed. A new form is submitted each school year. New forms will be available April 2025. Please contact Ms. Jessie Lawler, RN.
