Health Requirements
- Immunization records are due no later than Registration Day. Please review your child’s immunization history with your healthcare provider to ensure immunization requirements are up to date.
- All students need a complete physical exam within 12 months of the start of the new school year. The physical exam should be documented by your healthcare provider on this school health form or this school health form for sports.
- Please submit a record of the most recent physical exam no later than June 2, 2026.
Medication
- If your child takes or may need to take prescribed or over the counter medicine at school a medication administration form is needed. A new form is submitted each school year. New forms will be available April 2026. Please contact Ms. Jessie Lawler, RN.
