This policy is intended to notify you that Allied Counseling Services, LCC is a service provider of professional counseling services. State, Federal and local laws apply to ethical principles which require notification of factors involving your care and the relationship between yourself and Allied Counseling Services, LCC. Social media connections and other social media platforms are discouraged by ethical guidelines for the benefit of your professional services and confidentiality associated with your care. HIPAA compliant record keeping and communication is provided and your account and related records are securely stored and transmitted through contracts with private counseling portal, currently SimplePractice and email communication as well as messaging is done encrypted through Hushmail, alliedcounselingservices@hushmail.com.
Your counseling services are provided by a licensed Professional Clinical Counselor with a Supervisory endorsement as well as Independent licensure as Licensed Independent Chemical Dependency Counselor, both nationally and primarily in St of Ohio, USA. Consent forms are necessary for your services and for specific types of service, such as telehealth or video and phone services. Consents and questionnaires prior to sessions in-person may be required for COVID precautions according to guidelines for the same. Your counseling services are provided by Bryan Traband and he graduated with a Master of Arts from U of Toledo in 2010 with a Masters in Counselor Education. He is proficient in couples, addiction, mental health including anxiety, depression, crisis intervention, and problem gambling, internet addictions (process addictions). He utilizes Motivational Interviewing which is intended to empower you to make choices in your own best interest in order to address the issues you may be having at the time you reach out for help. CBT or cognitive behavioral therapy is intended to provide some accountability towards an agreed upon plan to further improve your quality of life by implementing your time on specific coping tools or efforts between sessions. Mindfulness based therapy is an evidence-based therapy which includes most aspects of therapy for a myriad of presenting issues and is provided throughout your therapy with Allied Counseling Services, LLC. Please understand that the quality of care you receive is important to Allied Counseling Services, LLC and your feedback is appreciated, both negative and positive, in order to improve the quality of care provided moving forward. My licensure requires on-going training and maintaining hours in order to provide quality and current forms of therapy for you, it is current and the following Identifications numbers apply for each, LPCC-S-E.080410-SUPV, and LICDC 111050.
You will also receive an additional file for consent to telehealth services if applicable. Additional attachments including release of information may be necessary at some point of your care. Any files requiring review and signatures will be provided through HIPAA compliant means as referenced previously. These may include letters on progress, diagnosis, treatment progress, and prognosis and be to a specific person (always requires a release). Worksheets or journaling tools, etc are also examples of HIPAA compliant care provided for you.
