ONSCREEN Pilot Program: Joy Calls for Southern California Seniors

Terms of Use and Consent Agreement

This document outlines the Terms of Use and Consent Agreement for participants in the ONSCREEN Joy Calls Pilot Program for Southern California residents. This program is designed to provide eligible older adults with free access to Joy Calls, an AI-powered telephone-based wellness companion service. Participation in the program constitutes agreement to the following terms:

1. Voluntary Participation
Participation in the Joy Calls Pilot Program is entirely voluntary. Eligible participants may enroll to receive Joy Calls at no cost for a period of up to six (6) months. Participation may be discontinued by the participant or by ONSCREEN at any time without penalty.

2. Eligibility This pilot is open to residents of Los Angeles, Orange, Riverside, San Bernardino, and San Diego counties. Participants must be age 65 or older, or be a caregiver enrolling on behalf of an older adult within those counties.

3. Survey Participation Participants (or their caregivers) agree to complete three brief surveys: one at the beginning of the pilot, one at the halfway mark, and one at the conclusion. These surveys are critical for assessing the effectiveness of Joy Calls in reducing loneliness, improving wellness, and supporting daily routines.

4. Call Activity and Data Collection Participants consent to the collection and analysis of basic engagement data, such as call frequency, type of check-in, and general sentiment or mood trends. This information is used to improve the service and evaluate the impact of Joy Calls. Any shared data will be de-identified and aggregated to protect individual privacy.

5. Privacy and Security
All collected data is used solely to enhance user experience and support caregiver or program insights. ONSCREEN uses secure, encrypted systems and follows best practices for data privacy.

6. Use Limitations and Disclaimer
Joy Calls is not a replacement for professional medical care, emergency services, or clinical diagnosis. It is designed to offer social support, reminders, and wellness check-ins. Participants understand that ONSCREEN and its affiliates are not liable for medical, emotional, or technical outcomes related to program participation.

7. Free Trial and Program Duration
Participants approved for the program will receive up to six (6) months of Joy Calls service at no cost. Access will be granted on a first-come, first-served basis and is subject to availability. ONSCREEN reserves the right to modify or end the program at any time.

8. Optional Media and Story Sharing
Participants may choose to share testimonials or general feedback for use in public awareness or promotional efforts. No identifying information (including names, voices, or personal details) will be shared without written consent.

9. Governing Law
This agreement is governed by the laws of the State of California. Any disputes will be resolved under the jurisdiction of California courts.

By participating in the ONSCREEN Joy Calls Pilot Program, you acknowledge that you have read and understood this Terms of Use and Consent Agreement and agree to abide by its conditions.
