Personal Information Protection Notification of ADI
告知事項 | Tips
數位發展部數位產業署（下稱本署）為了執行數位發展部數位產業署跨域資安強化產業推動計畫，將蒐集、處理及利用您的個人資料（下稱個資），謹先告知下列事項：
In order to collect, process, and use your personal information which you have provided or will provide (hereinafter referred to as “the Personal Information”), based on the reason that Cross-domain information security strengthening industry plan, ADI would like to inform you about the following matters:
一、蒐集目的：078 計畫、管制考核及其他研考管理
Purposes for collection:078 Planning, Audit Control, and Other R&D Management.
二、個資類別：C001 辨識個人者、C011個人描述、C012 身體描述、C013 習慣、C038 職業、C051 學校紀錄、C052 資格或技術、C061 現行之受僱情形
Classification of the Personal Information: C 001 Type for identifying individuals、C011 Personal description、C012 Body description、C013 Habits、C 038 Occupation、C051 School record、C052 Qualifications or skills、C 061 Current state of employment
三、利用期間：至蒐集目的消失為止。
Time period of the use of the Personal Information: until the purposes for collecting the information no longer exists.
四、利用地區：中華民國地區及本署辦事處所在地區。
Areas of use of the Personal Information: The territory of the ROC and ADI’s overseas locations and offices.
五、利用者：本署及其他與本署有業務往來之公務及非公務機關。
Users of the Personal Information: ADI as well as government agencies and non-government agencies that have or will have business relations with ADI.
六、利用方式：在不違反蒐集目的的前提下，以網際網路、電子郵件、書面、傳真及其他合法方式利用之。
Way of the use of the Personal Information: Under the condition that there is no excess of the scope of the purposes for which the Personal Information was collected, the Personal Information may be used on the Internet, in e-mail, in documents, in facsimiles, and in other lawful ways.
七、您得以書面主張下列權利：
（一）查詢或請求閱覽。
（二）請求製給複製本。
（三）請求補充或更正。
（四）請求停止蒐集、處理或利用。
（五）請求刪除。
若有上述需求，請與承辦人員 (電話：02-2700-0073；E-mail：service@hitcon.org ) 聯繫，本署將依法進行回覆。
You may exercise the following rights by raising written request(s):
1. any inquiry and request for a review of the Personal Information;
2. any request to make duplications of the Personal Information;
3. any request to supplement or correct the Personal Information;
4. any request to discontinue collection, processing or use of the Personal Information; and
5. any request to delete the Personal Information.

八、您若不簽署本告知暨同意書，本署將無法提供您特定目的範圍內之相關服務。
If you do not sign this Notification and Consent Regarding Collection, Processing and Use of Personal Information, ADI may not be able to offer a complete range of service(s) to you, and may also not be able to protect your lawful rights.
九、對本署所持有您的個資，本署會按照政府相關法規保密並予以妥善保管。
ADI will keep your Personal Information confidential and in proper custody in accordance with the relevant laws and regulations of the ROC.
