6th-grade Physical, Dental, and Immunization FAQs

What are the 6th-grade health requirements?

All students entering 6th grade will be required to submit, by August 1st, a physical on a Certificate of Child Health Examination form, and present proof of the Illinois Department of Public Health (IDPH) required immunizations.

6th-grade students must also have a Dental Exam submitted by May 15th of their 6th-grade school year. The dental exam may be dated on or after November 15th of the current school year.

Does my child need shots for 6th grade?

To avoid first-day exclusion, all students must present proof of the following IDPH-required immunizations by August 1st:

DtaP - 4 or more doses (with the last dose on or after the 4th birthday)

Polio - 4 or more doses (with the last dose on or after the 4th birthday)

Hepatitis B* - 3 doses

MMR - 2 doses

Varicella - 2 doses

Tdap* - 1 dose

Meningococcal* - 1 dose

*All students will show proof of receipt of three doses of Hepatitis B, one dose of the Tdap vaccine, and one dose of the Meningococcal vaccine with their 6th-grade physical.

My child had a physical in August of last year, and my insurance will not pay for another one until it has been 1 (one) year. What do I do?

Certificate of Child Health Exam forms dated within one year of the first day of school will meet the 6th-grade physical requirement. *Please note, students will still need to provide proof of immunizations.

Is a sports physical the same as a 6th-grade physical?

No. You may not use a sports physical as your 6th-grade physical. The 6th-grade physical must be on a Certificate of Child Health Examination form. If the box on page 2 of the form, next to “Interscholastic Sports” is checked “Yes”, then this physical may also be used as a sports physical. There is no need to have a separate sports physical if you have a 6th-grade physical with the interscholastic sports box checked “yes”.

My insurance has changed, or I don’t have a doctor.

The Community Resources for Physicals and Immunizations document was created to assist families with obtaining physicals and IDPH-required immunizations for their children. You may also use this guide for sick-child medical care. Please verify that the provider accepts your insurance coverage when making appointments.

What if my child isn’t 11 years old at their appointment time?

If your child’s physician determines they are not old enough to receive any of the vaccine(s), you must submit the completed physical with an appointment card indicating the date on which your child is scheduled to receive the vaccine(s). This should be no longer than 1 week after the 11th birthday.

If your child has not received the vaccine(s) by the date on the appointment card, they will be excluded from school until proof of IDPH-required immunization is presented to the school.

What if my doctor is out of a vaccine?

Make your appointments early to avoid any delay in receiving the IDPH-required immunizations. If your physician is out of a vaccine, you may use the Community Resources for Physicals and Immunizations document to assist you in finding another provider to give the vaccine.

Appointment cards will NOT be accepted if a physician's office is out of a vaccine.

When will an appointment card be accepted?

Appointment cards will be accepted for the following reasons:

• Your child is on a “catch-up” vaccine schedule.

• If your child is on a “catch-up” vaccine schedule, then the child shall present, by August 1st, a schedule for the administration of the IDPH-required immunizations and a statement of the medical reasons causing the delay.
  

• A delay due to a medical reason.

• If there is a medical reason for not receiving the vaccine. (ie. your child is sick at the appointment time or there are age restrictions), the child shall present, by August 1st, a schedule for the administration of the IDPH-required immunizations and a statement of the medical reasons causing the delay.

If your child has not received the vaccine(s) by the date on the appointment card, they will be excluded from school until proof of the IDPH-required immunizations is presented to the school.

When can I submit my physical?

NOW! WBO nurses are currently accepting all physicals and immunization forms for the 2025-2026 school year. All physical and immunization forms should be turned in as soon as they are completed and no later than August 1st. This allows the building nurses time to review your child’s health information and communicate missing or incomplete information back to you, avoiding potential first-day exclusions.

How can I submit my health documents?

Health information may be submitted by:

• Putting the documents in the dropbox located in the vestibule of the WBO main office

*Please note that nurses do not work over the summer. They will return on August 4th.
