I, the undersigned, give permission for the above named child to participate in the South Tama Recreation Program(s) chosen above.  I grant permission for my child to use the transportation system provided by a partnership with the South Tama School District.  I grant permission for emergency first aid to be administered to my child in the event of an injury or illness while under REC supervision.  I understand that the South Tama Recreation program is not responsible for accidents or injuries while participating in the REC program. I/We have read this agreement in its entirety and fully understand it.
