KODEX HEALTH INSTITUTE

 

History

Kodex Health Institute Basic Nursing Assistant Program was established in 2017, on the south side of Chicago. Kodex Health Institute has convenient access to many nursing homes and hospitals. The program will enable students to get hands-on training, in a vocational environment to become a certified nursing assistant. It is my passion to enable superb leadership to students to obtain this certification.

 

Organization

Students have hands-on experimental learning in both the nursing home and hospital setting during their training.  

The program is approved by the Department of Public Health. Upon successful completion of the course, the students receive a certificate of completion making him/her eligible to be employed in home health agencies, nursing homes, and hospitals. Students must also take a written competency exam as required by state and federal guidelines in order to complete the program.

 

Illinois Department of Public Health (IDPH) has set minimum competencies for CNA training in the state of Illinois. KHI (Kodex Health Institute) Basic Nurse Assistant (CNA) Program has chosen to set a higher standard than that required by IDPH. In order to complete KHI’s Basic Nurse Assistant Program, the student must meet KHI requirements.

 

 

 

                       Nursing Assistant Course Objectives

 

         Upon successful completion of the nursing assistant course, the student should be able to

 

1.      Identify the role and responsibilities of the nursing assistant as a member of the health care team in the home and health care facilities.

2.      Function safely in the role of a beginning nursing assistant under the supervision of a licensed nurse.

3.      Demonstrate appropriate knowledge, skills, and attitudes necessary to provide nursing assistant level care to people of various ages, racial and ethnic background at home and in healthcare facilities.

4.      Meet the requirements for and be eligible to take the required Nurse Aide Competency Exam, passage of which is necessary to be certified as a Nursing Assistant.

5.      Demonstrate a beginning understanding of the patient's physical, psychological, social and spiritual needs.

 

Academic Standards

Students are evaluated on personal qualities, nursing practice skills, attendance, and classroom/ clinical performances.

1.                   Personal qualities

Students must demonstrate certain personal qualities for dealing with patients and co­ workers: dependable, courteous, conscientious, cooperative, trustworthy, honest, respectful, caring, and enthusiastic. Appropriate behavior must be demonstrated at all times during class and clinical. Profanity is not to be used. Failure to exhibit proper behavior and/or language may result in dismissal from the course.

                                                                                        

2.                   Nursing practice

Students must maintain safe practice skills in the clinical area. Failure to maintain safe practice skills may result in immediate dismissal from the course, as may failure to report any accidents/injuries or incidents occurring to residents/patients in the clinical setting.

Patients and their families are not to be discussed outside the classroom. Personnel and/or policies of the cooperating agency are not to be discussed outside the classroom.

 

ANY STUDENT WHO VIOLATES CONFIDENTIALITY IS SUBJECT TO IMMEDIATE DISMISSAL.

 

Student Responsibilities

a.           As adult learners, students are expected to assume responsibility for their learning. The student is responsible for asking questions and requesting assistance or clarification from the instructor when needed. Students should check with their instructor about when he/she is available to help if you need assistance outside of class.

Any student who may need accommodation due to a disability should make an appointment to see the instructor during his/her office hours. A letter from the Special Needs Office authorizing your accommodations will be needed. Students requesting special accommodations (for example, a note taker, quizzes read, extended test nme, etc.) must formally request these services from the administrator .

c.           Students are expected to abide by all KHI policies as included in the school catalog and student handbook.

d.          It is the student's responsibility to notify the instructor of a name, address or phone number change that occurs during the course of the class.

4.                  Attendance

Regular classroom attendance and clinical attendance are essential. The two experiences are interrelated and absence from the classroom sessions will make you less effective in the clinical area. Students must attend 82 hours of classroom and 42 hours of clinical experience, as mandated by the Illinois Department of Public Health. A record of attendance is kept. By IDPH regulations, students MUST BE PRESENT for certain days of the course.

These days are listed in the reading schedule as "IDPH Required Attendance Day." Attendance on the first clinical day is also required. Students will be dropped from the course if absent on any of these days.

Classroom Attendance Policy

o        Students may not miss more than 6 hours of classroom time or they will be dropped from the program.

o        Attendance on "IDPH Required Attendance Day" as designated in the course reading schedule is required.

o        Students are required to notify their instructor if leaving class or lab, or they will be dropped from the course.

o        Sleeping in class is not allowed and students will be counted as absent.

Tardiness is counted in absence time and is defined as being one minute late. More than three episodes of tardiness, in class/clinical or combination of the two, during the course will result in dismissal from the course. If a student is tardy on an IDPH required attendance day, the time must be made up based on the availability of the instructor.

·         If class will be missed, a message should be left at 708 981 8847 or the number given to you by your instructor. Call in at least one hour before class starts and leave the following information in the message:

o        Your name and that you are an KHI  CNA student.

o        Explain the reason for the absence.

o        Your instructor may ask for written verification from appropriate persons.

                      Midterm and Final Exam: In the event of an emergency and a student is unable to take the midterm or final exam, the student MUST leave a message with the instructor prior to the start of the exam. Failure to notify the instructor will result in a 5% reduction in the student's score. Faculty may request written verification of the emergency.

If the final exam is not taken by the end of finals week, the student will receive an 'F' for the course.

Section Changes

Students are not allowed to attend classes (including clinical) in a section for which they are not registered unless it is cleared with the Associate Dean of Nursing. The ratio of students to instructor in the clinical area must remain fairly constant and orientation of a student from a different section to a new clinical area can create problems for students, agency staff and instructor. Often class sections do not follow the same format or content at the same time.

Attendance on First Clinical Day

Attendance on the first clinical day of this course is REQUIRED. No student may be absent on this day or they will be dropped from the program.

Attendance Policy for Subsequent Clinical Days

          Students with a verifiable and justifiable emergency may miss no more than 6 hours of clinical, per IDPH guidelines.

          The required procedure to follow if student will miss clinical:

          Call the day of clinical, at least one hour or more prior to the start of class.

          The clinical facility must be called and it is recommended that you call the school also. Leave the following information when calling in:

o        Identify yourself by name and that you are an KHI CNA student.

o        Explain the reason for the absence.

o        Your instructor may ask for written verification from appropriate persons.

There may be a rare instance where the above policy cannot be followed. In that case, the instructor must be informed within 12 hours of the start of clinical or the student will be considered a "no call/no show" and will be DROPPED from the course.

         Clinical Tardiness:

o        A student who is tardy more than 3 times in class/clinical or a combination of the two will be dismissed from the course.

o         If 6 hours of clinical have already been missed, and the student is tardy even one minute, the student will be dismissed for excessive absence hours.

Clinical Abandonment

o        Any student who leaves clinical without notifying the instructor will be dropped from the course. This constitutes abandonment.

These three requirements total 50% of the theory grade. You must achieve this requirement in order to take the final exam

50% OF THEORY GRADE

CLINICAL – Pass/Fail

If a student falls below a 75% on the final exam and the workbook  is not completed the student will be dropped.

 

Once a student falls below the required 75% grade for the course, the instructor will drop them from the course. It is the student's responsibility to contact registration and drop the course. If the student does not drop the course, they will be given an F for the class.

The following grade system is used in classroom theory: 75%-82%

A = 92% - 100%

B = 83% - 91%

C = 75% - 82%

F = 74% and below

 

All quizzes are required to be taken on the date and at the time scheduled.

Once the instructor has handed out the quiz, a late student will not be able to take that quiz and will receive a zero. There is not opportunity to make up a quiz if late or absent from class. No retakes are permitted.

Students must get 100% on the Vitals Quiz. The first grade goes in the grade book and students will retake it until they earn 100%. Vitals Quiz must be passed before the second day of clinical.

Any evidence of cheating is grounds for a failed assignment. Further action up to and including dismissal from NAS 101 may be instituted if deemed necessary.

You will be evaluated in two ways: a combination of your performance in the lab and actual clinical setting and on your performance on assignments, quizzes, and tests.

A midterm and final exam and evaluation will be given. No retakes are permitted.

A passing grade of 75% on the final exam is necessary for satisfactory completion of the course.

Workbook assignments one day late may be assessed a 25% penalty on that assignment Assignments two or more days late will receive a zero.

Students will be given one verbal, then one written warning for unsatisfactory/inappropriate behavior or performance. A third violation will result in dismissal from the program. Some infractions could result in immediate dismissal without a verbal or written warning.

 

According to federal legislation that gives guidance to the Illinois Nurse Aide Competency Evaluation, the following twenty-one selected performance skills must be passed successfully in order to complete the class. Those marked with an asterisk can only be tested in the clinical setting using a resident as the patient. All other skills can be tested in either the lab or clinical setting; except hand washing, which will be tested in both settings. The twenty-one skills are:

Skills will be demonstrated and practiced in the lab before the student 1s tested on them; regardless of where they are to be tested. Students are required to participate in classroom/lab activities.

Students are given 3 chances to correctly return demonstrations on the above skills. Practice outside the class/clinical setting is essential to being prepared for skills testing. If the student fails to properly return demonstrate all of the 21 skills in 3 attempts, they will be dropped from the class and assigned an 'F' grade. Students must demonstrate safety in all skills. Students must also demonstrate good body mechanics at all times.

At no time are students to use any type of mechanical lift without either their instructor, or a member of the facility staff who is trained in using the lift, present.

The final grade will be computed as follows:

15% workbook and written assignments

15% midterm

15% weekly quizzes

50% final exam

                                      Clinical performance

Written manual skills requirements are given to use as the basis for evaluation of performance. Using the manual skills, students must return demonstration skills in the lab area with a satisfactory "pass" before doing the procedure in the clinical area. In the clinical area, the student must consistently demonstrate and practice these skills in order to satisfactorily complete the clinical component of the class.

Maltreatment of patients (physically or verbally) can result in immediate dismissal from the program. Failure to maintain safe practice skills may result in immediate dismissal from the course, as may failure to report any accidents/injuries or incidents occurring to residents/patients in the clinical setting. Students will be given one verbal, then one written warning for unsatisfactory/inappropriate behavior or performance. A third violation will result in dismissal from the program. Some infractions could result in immediate dismissal without a verbal or written warning.

If you receive an "unsatisfactory" evaluation on your clinical performance, you will receive a final grade of 'F' regardless of your theory grade.("Failed" or "unsatisfactory" means the student does not demonstrate improvement. "Unsatisfactory" can mean injury or negligence by the student to a resident.) Evaluation forms will be explained by the clinical instructor.

 

                                               General Information

Dress code for classroom: Appropriate dress as allowed by instructor. No hats or headwear other than religious habit allowed.

Telephone calls and restroom stops are included in break time. Only emergency calls will be taken during class or clinical hours.

Smoking: KHI  facilities are smoke-free . Smoking is allowed in designated areas only.

Snacks or drinks are not allowed in the classroom without permission of the instructor. The only drinks that are allowed are ones with lids firmly attached.

Parking: Students must park in the appropriate student parking at the site of their class. They should check with their instructor if in question as to where to park.

Accident/illness: Students are responsible for their own health and safety during classroom, labs and clinical portions of the course. Students are to report any accident or illness to their instructor. The school assumes no responsibility for illness or injury occurring at the school or affiliated agency and no liability for medical expenses. These expenses would be the responsibility of the student.

Insurance: Students are responsible for their own health/accident insurance coverage. Student malpractice liability insurance is provided through the school and is included with tuition. This insurance is in effect while you are a KHI CNA students

No audible devices or telephones are allowed in the classroom during the class period. No text messaging during class or clinical sessions.

Alcohol and drugs: Drinking of alcohol or being under the influence is not permitted and students will be sent home. See the Substance Abuse Policy.

Change of Name/Address: Be sure to inform the class instructor of any name or address changes. In addition, a form must be completed and returned to the Office of Admissions and Records at KHI to report a name or address change. The form is available in the office or from your instructor . It is important to have the current address on the competency exam application form.

Sexual Harassment: Display of affection: Public display of affection between students at any time during the class or clinical portion of the program is strictly prohibited.

 

Technical Abilities required by the Basic Nursing Assistant Program: In order to handle the job responsibilities and tasks assigned to students in the Nursing Program, they must be able to:

1.             Perform a full range of body motion including handling and lifting patients, and moving, lifting, or pushing heavy equipment. Students are required to lift a minimum of SO# unassisted.

2.             Bend, reach, pull, push, stoop, and walk repeatedly throughout an eight hour period.

3.             Demonstrate visual acuity to read small letters and numbers on gauges (with correction, if needed).

4.             Demonstrate auditory acuity to hear sounds of blood pressures and apical pulses (with correction, if needed).

5.             Demonstrate bilateral upper extremity fine motor skills, including manual and finger dexterity and eye-hand coordination.

6.             Communicate in a rational and coherent manner both orally and in writing with individuals of all professions and social levels.

7.             Respond quickly and in an emotionally-controlled manner in emergency situation.

8.             Adapt effectively to environments with high tension.

9.             Maintain composure when subjected to high stress levels.

Sexual Harassment: Sexual harassment will not be tolerated. Students in the C.N.A. Program are required to abide by the College's sexual harassment policy as outlined in the school catalog. This policy applies to classroom and clinical settings. Also include is Chicago sexual harassment training.

Physical exam: The student must turn in physical exam (must use KHI form) stating student is free from communicable disease and has had two (2) negative T.8. skin tests (Mantoux) OR a chest x-ray. T.B. skin tests or chest x-ray must have been done within one year of the end date of the class. TB skin tests must be placed (given) 7-21 days apart. It is acceptable for the initial skin test of a 2-step procedure to be read when the second one is placed (given). HOWEVER, the second skin test MUST be read 48-72 hours after placement. If past positive, a copy of a current T.8. Checklist will be acceptable in place of a chest x-ray. A copy of a recent physical (within six months of start of class) will be accepted but must be submitted on an school form. This must be turned in to the instructor according to the date on the Reading Schedule and Dates to remember sheet. No student will be allowed on a clinical unit without meeting these requirements. NO EXCEPTIONS!!

NOTE: We require that all students are able to lift a minimum of 50 pounds unassisted. Students who are pregnant or who become pregnant by the time of the lab session, "Lifting, Moving, and Transfers as well as the clinical experience, must provide a signed release form from their physician in order to participate in these required sessions. This is for your own safety and well-being as well as that of the residents for whom you will be caring for.

Failure to provide this release form from their physician will result in dismissal from the program.

 

Students with injuries or illnesses requiring treatment while participating in CNA Program will be required to obtain a written medical release with no restrictions from the attending physician prior to returning to class or the clinical setting. NOTE: A discharge paper from the emergency room or clinic will not be accepted unless the physician issues a written medical release with no restrictions.

The Basic Nurse Assistant Program strives to maintain full compliance with the Americans with Disabilities Act (ADA) requirements. Any student with health concerns which could potentially interfere with their ability to complete the requirements of the Basic Nurse Assistant Program should immediately contact their instructor or the Program Coordinator.

Students with chronic health problems, including but not limited to diabetes, cardiac disorders, hypertension, epilepsy, skin condition or orthopedic limitations, are to be in a controlled state and free of symptoms.

Classroom: No children or pets are allowed in the classroom. Visitors in class will be allowed only at the discretion of the instructor and arrangements must be made prior to class.

Dress Code for Clinical Areas:

Students MUST:

1.             Wear clean and neatly pressed white skirts/pants, blue scrub top. We recommend cargo pants, especially for the men.

2.             Wear clean white leather uniform shoes (with clean laces where applicable) with brown nylon hose or white socks.

3.             Have hair neat and clean. Long hair must be pulled away from the face and secured.

4.             Wear a blue scrub top.

5.             Wear a watch with a second hand.

6.             Wear white undergarments

7.             Properly use body deodorants.

8.             Have fingernails fingertip length or shorter.

9.             All visible tattoos, except those on the hands, must be covered in the clinical area.

 

Students MUST NOT:

1.             Wear ribbons in hair.

2.             Have acrylic nails in the clinical area.

3.             Wear colored nail polish (clear or natural nail polish is acceptable).

4.             Wear jewelry (exceptions: wedding or engagement rings, "posts" for pierced ears, Medical Alert bracelets or Medical Alert necklaces). No visible body-pierced jewelry may be worn. One non-dangling post may be worn in each earlobe only. Any other piercings will need to be removed. They may not be covered with Band-Aids or spacers (retainers.)

5.             Use excessive perfume or makeup.

6.             Chew gum on clinical unit.

7.             Wear shorts, stretch-pants, leggings, Capri pants (peddle-pushers).

8.             Wear skirts shorter than 2" above the knee.

9.             Wear open-toed or open-heeled shoes (includes shoes with straps on back/heel.)

10.           Wear sweatshirts. Wear T-shirts with logos.

11.           Wear jeans (including white).

12.           Have unnatural colored hair; for example, blue, purple, green, etc., or any sprayed-in hair color.

13.           Wear thong underwear.

Failure to adhere to complete uniform regulations (including blue scrub top/shirt and watch) will result in being sent home from the clinical area and a clinical "unsatisfactory" will be given for that day.

Remember: a smile, a positive attitude, and professional conduct are the most important parts of the dress code. Wear them daily.

Other Clinical Requirements:

Promptness is equally necessary for clinical. Tardiness is figured the same as in class. Being present for pre- and post-conference is critical and is required.

 

Nursing Assistant students are to behave as ladies and gentlemen at all times. Respect for authority is to be demonstrated. Profanity is not to be used.

Do not bring valuables to the clinical area. Lock in your car or leave at home.

Students are required to have a black pen and a small pocket notebook for use in the clinical area.

Visiting patients at any other time in the capacity of a Kodex Health Institute nursing assistant student is strictly prohibited.

Every attempt will be made to have the student do their clinical in a facility other than the one in which they may be employed.

Occasionally, for unforeseen reasons, it may be necessary to stay a few minutes late in the clinical area to ensure patient safety and/or meet their needs.

No one, in the capacity of a CNA student, can give patient care without the instructor present in the facility

                                     Criminal Background Check:

Policy states that students must have a fingerprint background check. Students will sign a Disclosure and Authorization form on the first day of class. Fingerprints will need to be taken within ten (10) business days after the background check form is signed. There is a $25-$30 fee that must be paid at the time of fingerprinting. NO EXCEPTIONS.

Disqualifying convictions without a waiver will prevent a student from completing the course. (The following website has a list of disqualifying convictions unacceptable to Illinois Department of Public Health [IDPH] :http://www.idph.state.il.us/nar/disconviction.htm). If you feel that you have a disqualifying conviction, you should meet with your instructor privately as soon as possible after the start of class to discuss the matter. You should also call IDPH at 217-785-5133 to inquire about a waiver. If you need a waiver, you must withdraw from the class until such time as you have obtained the waiver. IDPH regulations state that a student with a disqualifying conviction and no waiver can complete a Basic Nurse Assistant course but cannot work as a Nursing Assistant without a waiver. In an attempt to save the student money and to comply with requests from clinical sites, KHI policy is that no student will be allowed in the clinical setting with a disqualifying conviction unless they have a waiver for that conviction. NO EXCEPTIONS. The college's policy supersedes IDPH policy.

Updates from Illinois Department of Public Health:

1.             CNA 's are required to notify the Health Care Worker Registry in writing (include phone number) of address and name changes within 30 days of the change. Mail to 525 W. Jefferson, 4th Floor, Springfield, IL 62761.

2.             CNA.'s who abuse, neglect or steal from their residents and it is substantiated by the IDPH will have the information placed on the Health Care Worker Registry, and that individual is permanently barred from working in Long-Term Care anywhere in the United States. If a CNA. is accused of abusing, neglecting or misappropriating funds from residents, they will be notified of the accusation by Certified Mail of a hearing. They will be granted the hearing ONLY if they respond to correspondence. Not responding will AUTOMATICALLY result in a GUILTY verdict on the charges, and the C.NA. will be permanently barred from working in long-term care anywhere in the United States.

3.             Reports of abuse, neglect, and/or theft can be placed anonymously to 1/800-252-4343.

4.             Regarding CPR training, even if an individual already has CPR certification and enters a nursing assistant training program, that person must still complete the 4 hour training requirement.

Substance Abuse Policy

The safety of clients and/or patients is greatly influenced by the cognition and behavior of the nursing students taking care of them. The nature of substance abuse includes denial and a pervasive inability to think and behave logically. It is the policy of KHI Nursing Program to require a urine and/or blood test immediately upon suspicion of substance abuse. The test(s) will be the student's responsibility. The student will need to arrange for transportation to the testing facility in the event they are required to go. The student must sign a release of information form allowing the results of the test to be released to clinical nursing faculty and/or the Associate Dean of the Nursing Program.

■              If a student refuses testing, they are immediately dismissed from the program.

■              if testing is positive; the student is dismissed immediately from the program.

■              If the student is to return to the CNA Program, they must have an alcohol/drug assessment at an approved program identified by the Associate Dean and sign a release of information permit for the Associate Dean to be sent the results of the assessment

*If the student is identified to have a drug/alcohol problem, they are referred for appropriate treatment. The student must present written proof of attending/completing appropriate treatment.

Readmission will only be considered if a treatment program has been completed and the student has met with the C.N.A. Program Coordinator.

It is the student's responsibility to ask his/her physician whether a prescribed drug may affect clinical performance.

Students must report the use of any prescribed drug or other substance that may impair clinical performance to their instructor. Failure to report use of such drugs or other substances or failure to provide proper evidence of medical authorization may result in the student's termination from the program.

Every student who observes or has knowledge of another student in a condition that impairs the ability to perform clinical duties or poses a hazard to the safety and welfare of others must promptly inform the instructor.

Nurse Aide Competency Exam

A student is eligible to take the written exam after they have completed Kodex Health Institute approved basic nursing assistant training program. Because applications cannot be submitted until the class is over, and since the testing program has specific dates by which applications must be submitted, it may not be possible for the competency testing to take place the next month following class completion. (See "Dates to Remember" sheet for the scheduled test date.) Check with your instructor to determine if there are sample tests available in the local Learning Resource Center.

 

1.             Complete the application form as provided to you by your instructor. Use a No. 2 lead pencil. Make sure you give accurate and complete information.

2.             The examination fee is $60 for first-time applicants. Give your instructor a MONEY ORDER or CASHIER'S CHECK (no personal checks or cash is allowable) made payable to SIU C. Anything other than a money order will be returned to you and may result in a month's delay in taking the exam. List your social security number on the money order. (Do not alter the money order in any way.)

3.             If you wish to take the competency test at another date you may bring the money order to our office at a later date. However, it is your responsibility to make sure you bring the money order to us in time to get it processed for the proper testing date. Refer to the following test dates/application

4.             SIU C will mail you a confirmation letter approximately one week before the test. You will be told the time of the testing and the testing room site.

5.             If you were scheduled to take the exam on a specific date (application and fee were sent to SIU C) and you were unable to take the test, you may re schedule for a fee of $20 for up to one year. If you re-schedule the exam after one year, the fee will be $60. SIU-C will mail you another application form to complete and return, along with the fee.

6.             Approximately two weeks after taking the written exam, you will be sent the results of the test from SIU C. Only a "pass" or "fail" is given. No number grades are given. A student who does not pass the exam has the option of two retakes within one year. A new application form must be completed and a fee of $30 will be charged. SIU-C will mail you another application form to complete and return, along with the fee.

d.             Scroll down near the bottom of the page on Health Care \\'orker Registry, and click on "Search the Health Care Worker Registry."

e.             Scroll down to the bottom of that page and enter name, then click "Search."

f.              If listed on the Registry, the name should come up.

g. Tuition Refund Policy

1.             Students who find it necessary to withdraw from the course must do so officially by obtaining and submitting forms from the Registration Office.

2.             Tuition will be refunded to students who withdraw officially according to the following schedule:

a)             Withdrawal prior to or on the 3rd day before the program begins( this does not include school closures).The refund will include 100% of the tuition, activity fees and class fees.

b)             No refund will be made for withdrawals for disciplinary reasons or absenteeism.

3.             Refunds due to students are made after the 10th day of the business period.

Reimbursement Scale

100% Refund: 100% refund is available until the 1st Friday of the 1st week of the start of classes

50% Refund: 50% refund is available until the 2nd Friday of the 2nd week of classes

25% Refund is available until the 3rd Friday of the 3rd week of classes

Students who withdraw after the date to get a tuition refund may qualify for a "W" [Withdrawal] grade, rather than an "F" in the course. To see if you qualify, contact the Registration Office as soon as possible and complete the appropriate process.

If an individual has certain criminal convictions the Health Care Worker Background Check Act, an Illinois state law, prevents many health care employers from hiring the individual as a direct care worker and in long-term care facilities from being hired as a worker who has or may have access to residents, their living quarters or their financial, medical or personal records. Some of the health care employers are: community living facilities; life care facilities; long-term care facilities; home health agencies; home health, home services, and home nursing agencies; hospice care programs; hospitals; assisted living and shared housing facilities; and other health care facilities listed in the Health Care Worker Background Check Act.

A waiver does not change your criminal record but it does allow an employer to hire you as a direct care worker or an access worker in long-term care.

Many considerations are taken into account when reviewing a waiver application.

•               Except in the instance of scheduled payments of court-imposed fines or restitutions, you must have met all obligations to the court and the terms of your parole (i.e. fines must be paid and parole, probation or mandatory supervised release successfully completed).

•               You must have satisfactorily completed a drug and/or alcohol recovery program if you were ordered to as part of the judgment.

•               Your age at the time of the offense, your work history, your criminal history in Illinois and other states, the amount of time since your last conviction, the severity of your conviction, and the circumstance surrounding your conviction, as well as other evidence that you provide are all considered in determining whether a waiver is granted.

•               You are less likely to have a waiver granted if you have several convictions in recent years or if your offenses were violent crimes. There are three categories of disqualifying offenses: Offenses that are always disqualifying except through the appeal process; offenses that may be considered for a rehabilitation waiver without a waiver application being submitted; and offenses that may be considered for a waiver by submitting a waiver application and additional required information.

You may have been convicted and not sent to jail. An individual may be fined, given probation or conditional discharge and it still be considered a conviction. If you are unsure whether an arrest or charge became a conviction, contact the circuit clerk of the county in which you were arrested.

If granted a waiver it is in effect until you are convicted of another disqualifying offense, which causes the waiver to be automatically revoked. Health care employers must check the Health Care Worker Registry (http://www.idph.state.il.us/nar) to see if you have met the requirements to be a certified nurse assistant and to determine if you have disqualifying offenses or a waiver, No other source of information (i.e. a waiver letter, certificate of achievement etc.) may be accepted.

If you have pending convictions or are about to have a conviction expunged or sealed, please wait until these processes are completed before applying for a waiver.

This is not meant to discourage you. There is a need for responsible, hardworking, and caring certified nurse assistants. However, if you have been convicted of a crime that disqualifies you from working for certain health care employers, you may want to seek a waiver before investing your time and money in training.

Crimes that disqualify (that means that you cannot work in a direct care position, like a CNA, with these convictions) you from working as a nurse's aide (these include both felonies and misdemeanors):

Please note: You may have been convicted and not sent to jail. Often people are fined or given probation, but these are still convictions. If you are unsure whether an arrest ended up to be a conviction, contact the County in which you were arrested and speak to the people at the Circuit Clerk or State's Attorney's office, or your attorney.

Some convictions that are not disqualifying (In other words, you can work if these are your only convictions):

·         Prostitution

·         Possession of cannabis or a controlled substance

·         DUI

·         Deceptive practices (writing "bad" checks on your own account)
