Data Collection: During the booking and consultation process, we collect personal information, including your name, contact information, medical history, and details relevant to your treatment. This information is essential to provide safe and effective care.

Data Usage: Your data is used strictly for the purposes of treatment, record-keeping, and communication related to your care. It may also be used anonymously for training and audit purposes, but only with your explicit consent.

Data Sharing: We do not share your personal information with third parties unless:

- You provide explicit consent for sharing,

- It is required to provide you with integrated care, such as coordination with other healthcare providers (in which case, consent will be sought),

- We are required by law to do so, or

3. 48-Hour Cancellation and Rescheduling Policy

Our clinic operates on a schedule designed to maximize availability for all patients. A missed appointment or late cancellation can prevent others from accessing care.

Cancellation Notice: We request a minimum of 48 hours' notice for cancellations or rescheduling. This allows us to offer the slot to other patients.

Late Cancellations: If you cancel within 48 hours of your appointment, a cancellation fee of the full appointment cost may be applied. This fee may be charged to the payment method provided at booking or collected at your next appointment.

No-Show Policy: Failure to attend your scheduled appointment without any prior notice may result in the full session fee being charged.

Rescheduling: We will do our best to accommodate rescheduling requests. However, repeated last-minute changes may result in restricted booking privileges. If you have previously moved your appointment within the 48-hour cancellation window, the charge will be carried over to your next session. Cancellation of this session will result in a charge for the original session that was moved.

Exceptions: We understand that unforeseen circumstances may arise. If you experience an emergency or illness, please contact us as soon as possible, and we will do our best to accommodate your situation.

4. Insurance claims and cash back plans

It is the patient’s responsibility to check with their insurer they are eligible for treatment of whichever service they require and that the qualifications held by our practitioners meet the requirements of the insurer.

For direct claims please provide the clinic with your membership number, authorisation code and excess amount before attending your appointment (AXA only)

5. Consent to Treatment

Your informed consent is a key part of our commitment to providing safe and respectful care.

Informed Consent: By booking and attending an appointment, you agree to undergo an osteopathic assessment and, if appropriate, treatment. Your osteopath will explain the nature, purpose, potential benefits, and any potential risks or side effects associated with the treatment. Consent will be obtained verbally or in writing prior to any intervention.

Right to Decline Treatment: You have the right to refuse or stop treatment at any stage, even after giving initial consent. Should you feel uncomfortable at any point, please inform your osteopath, and they will discuss alternative options.

Parental Consent for Minors: For patients under the age of 18, a parent or legal guardian must provide consent for treatment and attend the consultation session with the patient.

Medical History Disclosure: To ensure safe and effective care, we request that you disclose any relevant medical history, ongoing treatments, or medications. This information is kept confidential and is only used to inform your care.

6. Reassessment Policy

To ensure safe, effective, and clinically appropriate care, a reassessment may be required under certain circumstances.

Reassessment After Extended Time: If you have not attended the clinic for 9 months or more, you will be required to undergo a reassessment before further treatment can continue. This allows your osteopath to review any changes in your health, medical history, lifestyle, or symptoms that may influence your care.

Reassessment for New Injuries or Conditions: If you present with a new injury, new symptoms, or a condition unrelated to your previous course of treatment, a reassessment will be necessary. This ensures that the appropriate diagnosis and treatment plan are established for your current presentation.

Reassessment Fees: Reassessment appointments are charged at the initial consultation rate unless otherwise stated at the time of booking

7. No Guarantee of Treatment Success

While we aim to provide the highest standard of care, osteopathic treatment does not come with a guaranteed outcome. Every individual’s response to treatment is unique, and multiple factors may influence results.

No Assurance of Results: By agreeing to treatment, you acknowledge that AG Osteopathy and Sports Clinic cannot guarantee a specific outcome or degree of improvement. Treatment effects vary, and additional sessions or alternative therapies may be recommended depending on your progress and response.

Open Communication: We encourage open communication about your symptoms, progress, and any concerns throughout your treatment. If adjustments to your treatment plan are needed, your osteopath will discuss these options with you.

Alternative Treatment Options: Your osteopath may recommend complementary or alternative therapies if deemed beneficial for your condition. Please note that referrals or recommendations do not imply a guarantee of success.
