Motivation Worksheet: Substance Use Motivation Scale

All behaviour change requires some level of motivation to give one the push they need. On the scale below mark where you currently stand with respect to your motivation to end your substance addiction and set yourself on the road to recovery.

On the scale 0 refers to ‘’no motivation to change’ and 10 refers to ‘absolute final decision to quit’

 

Explain your choice in the space below. Explain why you did not choose a number higher or lower. Write down what have been your motivations in the past few days leading up to your decision to seek help for your problem.
