Accessing Your Magnus Health Account & Magnus 911

To access your Magnus Health Account:

To access Magnus, please navigate to your Veracross parent portal at https://portals.veracross.com/nshahs/ and login with your Veracross Username and Password. This is the same username and password you used when you applied for the school and that you use when you access report cards. In the menu bar, click on “Health Forms & Information”.

Then click on the Orange button labeled “Health Documents”. This will automatically direct you to the Magnus Health Portal.

You will then be automatically directed to the Magnus Health Portal which will look like this:

Click “Complete now” to get started. You will see a list of requirements that need to be completed. This is a sample of what your dashboard may look like depending on the completion status of each of your items.

A few important notes.

  1. Conditional Questions must be completed first. Your answers to these questions will determine what additional forms may be required for your child.

  1. The Vital Health Record contains many items that need to be filled out. Some of the items will already be pre-filled from your Veracross account. If there is a required field that you CAN NOT edit that means that the information needs to be updated in Veracross. Contact the school at 516-487-2424 x1109 to get assistance.

        The blue text boxes at the top of each section will provide guidance on what needs to be completed.

        On the left you will see details on next actions that may need to be taken.

        When you have completed all sections in the Vital Health Record, click Submit for Approval. Then click Close and save for later to go back to the dashboard.

        [NOTE: In the Health Emergency Contacts section, there is a question about whether you want to opt-in or out to Magnus911. Please see the description below explaining Magnus911 before answering this question.]

  1. Forms that require a physician's signature or hard copy. Some forms that you must complete, must be downloaded, filled in and uploaded back to the Magnus Health portal. For these forms you will see an icon that looks like this:

next to the form in the requirements list. In that case, you can view the form either by clicking that icon.  You will need to print the form and fill it out and sometimes have the physician sign, date & stamp it. When these steps are completed, you will then need to click Edit and upload the file. Click the Add Files button to upload. [NOTE: Do NOT click Clear Answer as this will remove any previous files unless you intend to replace an old one].

You can choose to scan the file onto your computer or take a picture of the file and send it to yourself. Then choose the file from the location where you stored it. Alternatively, if you have set up the mobile app, you can take a picture of the form and upload it directly from your phone.

Some of these forms are only required if it applies to your child. In that case, there will be Yes or No buttons for you to select. If you select No, then you do not need to complete the form. If you select Yes, then  you will need to complete the form.

  1. Annual Physical Exam Form. This form is an annual form and expires after one year from the last exam date. If you have a student new to NSHAHS you will need to upload this form. If you have a current NSHAHS student, then existing forms have been uploaded already and you should see the date that the form is approved until. You will need to upload a new form prior to the date listed.

  1. Immunization Form. This form is required for all students. If you have a student new to NSHAHS you will need to upload this form. If you have a current NSHAHS student, then existing forms have been uploaded already. If the form has been approved then you are all set. If not, then your child is out of compliance on one or more immunizations which you will need to address and re-upload a new immunization form.

        You may also receive a notification directly from Magnus if your child is non-compliant.

        It is in your best interest if you advise your child’s physician to ensure that all administered immunizations have been entered into both the NYS and the City-wide immunization registries as this is where Magnus Health is confirming your child’s record of immunization.

Completion

When you have completed all forms, you will see a message at the top right that you are complete.

Please note that some of the requirement items may show a yellow triangle with an exclamation point if the forms require school approval. Once they are approved, it will show a green circle with a check mark.

You will be able to see in your dashboard any items that do not get approved and you will need to resubmit the forms that need to be adjusted.

At any time during the year, you may access your Magnus Health file to review your child’s medical record and make changes as necessary.

Magnus 911

One of the questions in the portal you will be asked is whether you want to opt in to Magnus 911. Magnus 911 is a card or piece of paper that you can take to an off-campus event with student health and demographic information. In case of an emergency, a school can keep printed Magnus 911 cards at the health office or the front desk. If a school needs this information for a specific student, we supply a clean one-sheet page with critical health information such as contact information, allergies, and medications. We allow you to print this out per class, so if the entire class is traveling to Disney World, you may print a pdf for each student going.
